Serenity Hospice and Home provides an Interdisciplinary Team of trained hospice personnel to create and maintain a plan of care. The care plan is developed to meet each patient’s individual needs for pain management and symptom control. The team consists of:

Hospice provides all palliative medications for the alleviation of pain and suffering. Appropriate drugs are used to control pain, relieve nausea, promote rest, ease breathing difficulty, and minimize other distressing problems. Serenity Hospice & Home works with a specialized pharmacy. The pharmacy provides compounded drugs which are medications prepared in gel form that can be rubbed into the skin if swallowing becomes difficult. The medications are delivered directly to wherever the client calls home, including private residence, nursing facilities, or Serenity Home. Vitamins and previously used medications (not related to the terminal illness) are not covered as part of the hospice program.

Hospice provides all equipment to promote a safe and comfortable environment for the patient. Oxygen, walkers, wheelchairs, electric beds, air mattresses, and other supplies are leased and delivered directly to the home.

Serenity Hospice & Home has been providing care since 1984. We serve the counties of Boone, Carroll, DeKalb, Lee, Ogle, Stephenson, Whiteside, Winnebago, as well as the towns of Walnut and Ohio in Bureau County.
