Serenity Hospice & Home

Serenity provides ongoing palliative and end-of-life hospice care with the utmost care and dignity. Our services address the physical, spiritual, and emotional needs of the family unit, with an emphasis on the patient’s and family’s wishes. Serenity Hospice & Home believes life continues until the moment of death, and our hospice focuses on promoting optimal living throughout this final journey.

Serenity is a FIVE-STAR-rated organization from the Centers for Medicare and Medicaid Services (CMS). As a non-profit serving multiple counties, we thank the many donors and volunteers who help Serenity provide the best care available.

Serenity serves Rockford, Belvidere, DeKalb, Freeport, Lanark, Mt. Carroll, Byron, Rochelle, Oregon, Mt. Morris, Polo, Dixon, Sterling, Rock Falls, and more towns throughout the Northwest Illinois counties of Boone, Carroll, DeKalb, Lee, Ogle, Stephenson, Whiteside, and Winnebago, as well as the towns of Walnut and Ohio in Bureau County.
