Serenity Hospice and Home is looking for a compassionate and dedicated Registered Nurse with a passion for providing exceptional care! We are seeking a Hospice Admissions RN to join our caring team and make a difference in the lives of patients and their families. This salaried position operates from 8:00 am to 4:30 pm, Monday through Friday, while covering one Saturday and one Sunday every five weeks. Admissions may take place in our 8-bed in-patient unit or in a patient’s home. Mileage is reimbursed. 

Key Responsibilities:

• Conduct comprehensive assessments of patients referred to hospice care, ensuring a seamless and supportive admission process.
• Collaborate with interdisciplinary teams to develop personalized care plans that honor the unique needs and wishes of each patient.
• Provide education and emotional support to patients and their families, helping them navigate the transition to hospice care with dignity and understanding.
• Maintain accurate and timely documentation in accordance with regulatory and organizational standards.

Qualifications:

• Current RN license in good standing.
• Experience in hospice or palliative care preferred, but not required.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• A compassionate and empathetic approach to patient care.
• Detail-oriented individual who is highly organized.
